Immediatley make arrangements to provide the injured employee with appropriate medical care by calling hte Managed Care Organization (QUALCARE: 800-425-3222).
Effective October 1, 2008, injured workers may file an application for emergent medical treatment based on "irreparable harm". Once an employer receives services of this application, a response must be made within five calender days. Therefore, it is critical that you immediately phone your claims administrator upon receipt of this application (see claims contact numbers above).
New Jersey requires the electronic filing of workers' compensation reports. The First Report of Injury (FROI) can be downloaded using the following link under Client Access tab. Complete this form on ALL cases where the employee sought more than first aid treatment regardless of whether the employee lost time from work:

Any notice of intent to file a claim or summons and complaint must be forwarded within 24 hours to your designated claims administrator and your local risk management consultant. Also, advise the claimant that a tort claim notice must be completed by any individual seeking to assert a claim. Remediation costs to property under your care, custody and control resulting from negligent acts of a third party
DeMinimis abandoned toxic waste site liability
Comprehensive legal defense in covered claims
Third party liability and corrective acion plan coverage for scheduled storage tanks
Third party liability losses caused by an environmental impariment from such locations as scheduled recycling centers and foreclosed properties
Any environmental incident shoudl be immediately reported to First Environment at 908-303-3218. First Environment will dispatch an environmental engineer 24/7 to assist you. Examples when First Environment should be notified are as follows:
Any incident involving releases, leaks, spills, or other problems involving the storge of hazardous materials in an underground or above ground storage tank.
Any spill or release of hazardous materials or petroleum products to the soil that significantly threatens soil contamination.
Any discovery or suspicion of a realease of hazardous contamination to the groundwater.
The discovery of known or unknown hazardous material that requires the classification or identification of suspect chemical.

PUBLIC OFFICIAL/EMPLOYMENT PRACTICES (POL/EPL)

Public Officials - Claims arising out of an alleged wrongful act of a public official. Many of these claims arise out of issues involving planning and zoning boards, ordinances, and violation of civil rights.
Employment Practices - Claims arising out of an alleged wrongful act in connection with dismissal, work place harassment, failure to promote, etc.
The POL/EPL Policy and the Excess POL/Policy are claims made policies, which means all claims must be reported within the policy period and in accordance with the policy’s terms.Therefore, prompt reporting is essential and must be in accordance with the terms and conditions set forth below.
Upon the receipt of a “Claim” (which is defined in the POL/EPL Policy), the member shall report the Claim to Scibal Associates, your local JIF Claims Administrator.Scibal Associates shall immediately review the Claim and report it to the carrier’s claims adjuster, Summit Risk Services (“QBE/ Summit Risk Services”) with a copy to the Fund Attorneys (Robert Budesa and Mary Jane Lidaka), and Executive Director (Cate Kiernan).
The POL/EPL Policy requires each member to report “any incident, event, occurrence, loss or accident that might give rise to a Claim.Such report of a potential claim must be completed immediately in accordance with the policy terms.Each member should report potential claims in the same manner as a Claim, as set forth above.
Please note that early reporting of facts and circumstances that might give rise to a claim is required by the POL/EPL Policy and failure to do so could result in loss of coverage in the future.Experience has demonstrated that reporting such potential claim could result in early resolution with the occurrence of no expenses.QBE/ Summit Risk Services will offer assistance with early resolution of Claims and potential claims that are reported to them in a timely fashion.
PROPERTY AND VEHICLE LOSS
Universal Property Loss Claim Form: Complete this form and email or fax it to the claim administrator. This form is only to be used for claiming damage to YOUR OWN property.
Types of Property Losses to be Reported on this Form:
Damage to your building or its contents
Damage to equipment you own or lease and are required to cover
Damage to any auto you own or lease
Damage to vauable papers
An interruption of services on revenue producing property
Damage to fine arts owned and non-owned
Damage to a building owned by you and under construction
Damage to a watercraft
Damage to backhoes and off road equipment
Burglary, robbery, theft of funds and fidelity losses
Depending on the circumstances, NJUA also covers damage to objects ushc as air conditioning systems, boilers, electrical lines and connections, or machinery such as deep well pumps, transformers, and production equipment caused by an accident that results in mechanical breakdown, electrical damage or steam explosion.
Report losses to the NJUA claims administrator on the Universal Property Claim Form. Significant claims should be immediately reported by telephone.
